Eligibility for the Multipurpose Senior Services Program (MSSP)

You may be eligible to participate if you:

  • Are 65 or older, frail or disabled
  • Live in Oakland/Emeryville/Piedmont or Berkeley zip codes 94704 or 94705
  • Receive monthly Medi-Cal benefits
  • Are at risk of nursing home placement because of frailty or inability to manage without help
  • Are willing and able to accept care management

  • Referral to home-delivered meals
  • Respite care
  • Referrals to medical care
  • Non-medical home equipment
  • Referrals to advocacy counseling
  • Referrals to legal services

  • A visit by a social worker and a nurse to evaluate you in your home
  • A care plan developed with you, your family and other caregivers to address and identify your physical, emotional, social and environmental needs
  • Referral to and purchase of services and advocacy, if needed, to help you stay safe and independent in your home
