If you have proof of your public benefit, choose Option 1 in Step 2.
If you do not have proof of a public benefit, choose Option 2 in Step 2. Follow the steps on Page 3 of the form to verify your income level or apply for CalFresh/Medi-Cal.
What to mail
If you chose Option 1 in Step 2, include a copy of your Public Benefit card
If you chose Option 2 in Step 2, include the signed and dated Page 3 of the form as well as any specified supporting documents.

What to bring
If you chose Option 1 in Step 2, bring the Speeding Ticket, bring the completed form and your benefits card to the City of Oakland Parking & Mobility Assistance Center.
If you chose Option 2 in Step 2, bring the Speeding Ticket, the completed form with Page 3 signed and dated, and specified supporting documents to the City of Oakland Parking & Mobility Assistance Center
