The Office of the City Clerk offers weekday passport application acceptances by appointment only. Our office provides the necessary forms, as well as friendly, professional acceptance agents. Our regular hours are Monday through Thursday: 9:00 am to 12:00 pm (last appointment at 11:30 am), 1:00 pm to 4:00 pm (last appointment at 3:30 pm) and Friday from 9:00 am to 12:00 pm ONLY.

Accepted Forms of Payment

Fees being submitted to the US Dept. of State MUST be in the form of a check or money order. No Cash, Credit, or Debit Cards accepted. Acceptance facility fees can be paid by check, money order, or credit card. Acceptance Facility Fees: $35 per application, $25.70 to overnight application(s).

Please note, following the US Department of State regulations, separate checks or money orders are highly recommended for each passport application.

Passport Checklist

All Applicants

  • Completed DS-11 Form in BLACK INK only (Acceptance facilities only process DS-11 applications)
  • Personal check, cashier’s check, or money order for US Dept of State fees
  • A previously issued passport, Certificate of Naturalization, or an original or certified copy of birth certificate
  • Valid Government-issued photo ID (of applicant and/or both parents/guardians)
  • Please see above for fees and accepted forms of payment
  • Passport Photos

Applicants Under 16 Years of Age

In addition to the items listed above:

  • Applicants 15 years or younger must be accompanied by both parents/guardians at the time of application or must have signed, notarized consent form (DS 3053) from the non-applying parent. Parents/guardians must supply valid government-issued identification.
  • First time applicants – original or certified copy of birth certificate
  • Renewals – previously issued passport and a photocopy of the birth certificate
