The Office of the City Clerk offers weekday passport application acceptances by appointment only. Our office offers the necessary forms, expedited service as well as friendly, professional acceptance agents. Our regular hours are Monday through Thursday: 9:00 am to 12:00 pm, 1:00 pm to 3:30 pm and Friday 9:00 am to 12 pm ONLY. We also offer after-hour passport application acceptance hours once a month. Please check the City Clerk’s home page for upcoming dates and times.

Your appointment will take place at: Office of the City Clerk, 1 Frank Ogawa Plaza, 2nd Floor, Oakland, CA 94612.

Accepted forms of payment: Fees being submitted to the US Dept. of State MUST be in the form of a check or money order. No Cash, Credit or Debit Cards accepted. Acceptance facility fees can be paid by cash, check or money order. Acceptance Facility Fees: $35 per application ($25.70 to overnight applications to the Department of State)

Passport Checklist

All Applicants

  • Completed DS-11 Form in BLACK INK only (We only process DS-11 applications)
  • Personal check, cashier’s check, or money order
  • Old passport, Certificate of Naturalization, original or certified copy of birth certificate
  • Valid Government-issued photo ID (of applicant or both parents/guardians)
  • Passport photos
  • Please see above for fees and accepted forms of payment

Applicants Under 16 Years of Age

In addition to the items listed above:

  • Applicants 15 years or younger must be accompanied by both parents/guardians at the time of application. Both parents/guardians must supply valid government-issued identification.
  • First time applicants – original or certified copy of birth certificate.
  • Renewals – previously issued passport and a photocopy of the birth certificate.
